Requirements

• Original and duplicate (or photocopy) of the document/s to be acknowledged.  The Embassy will retain the duplicate.

• Personal appearance of the executor / signatory of the document.  The original document must be signed in the presence of the consular officer / staff.

• Photocopy of the executor / signatory’s valid identification which bears his/her photograph and signature (passport, residence card, driver’s license, etc.)

• Cash payment for processing fee of each document (refer to Consular Fees)

The acknowledged or notarized documents signed by the consular officer may be collected after two (2) working days.
