REQUIREMENTS FOR THE PETITION FOR CORRECTION OF CLERICAL ERROR IN CIVIL REGISTRY DOCUMENTS REGISTERED AT THE PHILIPPINE EMBASSY PURSUANT TO R.A. 9048/10172:
- Duly accomplished application form;
- One (1) original and three (3) copies of the civil registry document (Report of Birth/Report of Marriage/Report of Death) to be corrected issued by the Philippine Statistics Authority (PSA);
- At least two (2) public or private documents showing the correct entry/entries upon which the correction or change shall be based (the original or a certified true copy of the document must be presented);
- Other documents which the Consul General may consider relevant and necessary for the approval of the petition;
- Copy of passport or any government-issued identification card of the petitioner; and
- Applicable fees of Euro 55.00 or Euro 165.00.
