These requirements are for children born in Spain of Filipino parent/s.
Basic Requirements
The Report of Birth form must be accomplished by either of the Filipino parents or the legal guardian, and the child must be brought to the Embassy or Consulate.
- Duly completed Report of Birth form in five (5) originals
- Original and five (5) photocopies of the following documents:
- Multilingual version of birth certificate (Certificado Literal de Nacimiento) issued by the Spanish Civil Registry (Registro Civil). If the multilingual version is not available then an official English translation will be required.
- Passports of parents and photocopy of the datapage
- Marriage contract of parents (if applicable) issued by PSA or multilingual version of the marriage certificate (Certificado Literal de Matrimonio) issued by the Spanish Civil Registry (Registro Civil). If the multilingual version is not available, then an official English translation will be required.
- Affidavit of Late Registration and Certificate of No Birth from PSA for children more than 12 months old
- Cash payment for application and processing
Additional Requirements (for new-born babies whose parents are not legally married)
- Affidavit of Acknowledgement of Paternity to be executed by the father or private handwritten instrument
- Affidavit to Use the Father’s Surname (AUSF)
- Cash payment for processing
The Embassy/Consulate will forward the Report of Birth to the Department of Foreign Affairs for onward transmittal to the Philippine Statistics Authority (PSA) in the Philippines. Applicants may retrieve the PSA-certified birth certificate in the Philippines a minimum of three (3) months after reporting the birth to the Embassy.
LEGITIMATION OF BIRTH
Requirements:
Due to subsequent marriage of parents:
-PSA-issued Marriage Advisory Certificate/Certificate of No Marriage of both parents
-Original and photocopy of PSA-issued Report of Birth
-Affidavit of Legitimation executed by both parents
-Affidavit of Acknowledgment of Paternity
-PSA-issued Marriage Certificate
-Child's Literal de Nacimiento (with English Translation)
If parents are not Married but father acknowledges the child and allows his Last Name to be used by the Child
-Affidavit of Acknowledgement of Paternity
-Affidavit to Use the Surname of the father executed by both parents
-Original and photocopy of PSA-issued Report of Birth
-Child's Literal de Nacimiento (with English translation)
