Basic Requirements
- Personal appearance of the applicant
- Duly completed Passport Application form; Adult Renewal, Adult New, Minor Renewal or Minor New
- Affidavit of Loss or Damage to be executed at the Embassy
- Cash payment for passport replacement and notarization fees (refer to Consular Fees)
Additional Requirements

- Original and photocopy of Birth Certificate issued by PSA (if) Marriage Certificate issued by PSA/NSO (if married and using the LAST NAME of the husband)
- Photocopy of lost passport
- Written police report (denuncia)
- Residence permit: NIE ( if available)
- Pardon municipal de habitants certificado de inscripción
Other official identification documents (driver’s license, voter’s ID, seaman’s book, etc.) that prove the applicants identity.
Note: There is a 15-day clearance period for investigation and approval from the DFA for lost passport applications. A new passport may only be issued after the DFA has given an official endorsement / approval and the applicant has complied with all the documentary requirements.

Passport processing takes 4 – 6 weeks. As a passport with at least 6 months’ validity is a requirement for international travel, it is advisable that applicants renew their passports early to avoid inconvenience. If there are less than 6 weeks until the date of travel to the Philippines, it may be possible to apply for a Travel Document.
Basic Requirements

- (For children, the form must be completed by either of the Filipino parents or the legal guardian and the child must be brought to the embassy.)
- Original and photocopy of passport (for renewals)
- Cash payment of passport renewal fee (refer to Consular Fees)
- Photos are no longer required, it will be taken digitally upon application, but applicants are required to wear the following:
ü For men: decent shirt with collar
ü For women: decent attire, with or without collar; no sleeveless shirts, no plunging necklines.
ü Note: DO NOT wear earrings or necklace when applying for a passport.
Additional Requirements
For Minors (Proof of filiation):
- PSA-issued Certificate of Live Birth, Report of Birth, or Certificate of Foundling
- Personal appearance of one of the parents named in the PSA-issued birth certificate
- Valid Philippine passport or other competent proof of identity of the accompanying parent and the copy of his/her spouse’s passport or other competent proof of identity
- If the accompanying parent is an alien, the non-Filipino parent must present his or her foreign passport and a copy of the Filipino parent’s Philippine passport or other competent proof of identity;
- If accompanied by an authorized adult companion, a Special Power of Attorney (SPA) executed by either of the minor’s parents or his/her legal guardian, as the case may be, his/her passport or other competent proof of identity, and a copy of the passport or other competent proof of identity of the minor’s parents
- PSA-issued Marriage Certificate if father is accompanying the minor applicant
ü If the parents are not married, only the mother shall principally accompany the child and file the passport application. Provided, that if the child is below seven (7) years old, the consent of the mother shall be required. Provided further, that custody has not been transferred to the father or a legal guardian by a competent court.
For Permanent Residents in Spain:
- Original and photocopy of Tarjeta de Residencia
For Dual Citizens (those who have taken their oaths of allegiance under RA 9225):
- Original and photocopy of Oath of Allegiance, Order of Approval and Identification Certificate
For Seamen:
- Seaman’s Book
- Crew list
- Letter from the master of the vessel / authorized agency
For Newborn Babies (refer to Report of Birth):
For Married Women Who Wish to Use the Surname of their Husband:
Married in Spain:
- Original and photocopy of Report of Marriage form filed at the Philippine Embassy
- Original and photocopy of multilingual version of marriage certificate (Certificado Literal de Matrimonio) issued by the Spanish Civil Registry (Registro Civil). If the multilingual version is not available then an official English translation will be required
- Photocopy of the data pages of the passports of both husband and wife
Married in the Philippines:
- Original and photocopy of marriage certificate issued by the PSA
- Photocopy of the data pages of the passports of both husband and wife
Married Abroad (countries other than Spain):
- Original and photocopy of marriage certificate issued by the PSA
- Photocopy of the data pages of the passports of both husband and wife
For renewal of Green or MRP passports (non-Epassport):
- Actual MRP/Green passport
- PSA – Authentic Birth Certificate
- At least one (1) valid identification document
- Other support document as necessary (i.e. PSA MC if married and using married surname, etc.)

HOW TO GET A PASSPORT APPOINTMENT
- STEP 1: To open the site for the Global Online Appointment System (GOAS) please click this link.
- STEP 2: Press “Schedule an Appointment”, read the Terms and Conditions and press either “Start Individual Appointment” or “Start Group Appointment”
- STEP 3: Fill out Site Location with the same details listed below and click NEXT:
Region – Europe
Country – Spain
Site – PE MADRID - STEP 4: Choose your preferred date and time of appointment, tick the “I am not a robot” box and click NEXT.
- STEP 5: Fill out the form with your Personal and Family Information.
- STEP 6: Fill out the section with your Application and Contact Information.
- STEP 7: Once the information required is duly provided, a “Notice of Provisional Schedule” will be sent to the applicant’s email.
- Kindly confirm the appointment WITHIN ONE (1) HOUR upon receipt of the notice. Failure to do so will automatically cancel the booking.
- STEP 8: Once your booking is confirmed, you will receive another email with the duly accomplished passport application form. Kindly print and bring the passport application form during your appointment date, together with the original and photocopy sets of the documentary requirements.
